Organizational regulations of the medical entity MABEL HAIR CLINIC Spółka z ograniczoną odpowiedzialnością. Last update: August 2026.

General Provisions

§ 1

  1. These regulations have been adopted for the entity performing medical activity, conducting business under the name MABEL HAIR CLINIC Spółka z ograniczoną odpowiedzialnością with its registered office in Warsaw, ul. Świętokrzyska 30/63, 00-116 Warsaw, NIP: 5253013662, REGON: 529404400, hereinafter referred to as “MABEL HAIR CLINIC”.

  2. The above-mentioned entity was entered into the Register of Entrepreneurs by the District Court for the Capital City of Warsaw, 12th Commercial Division of the National Court Register under KRS number 0001121972.

  3. MABEL HAIR CLINIC is entered in the Register of Entities Performing Medical Activity, kept by the Governor of Mazovia under Book No. 000000288582, Authority designation W-14.

  4. MABEL HAIR CLINIC operates on the basis of applicable legal acts, including in particular: the Act of 15 April 2011 on medical activity (consolidated text: Journal of Laws of 2025, item 450) and implementing acts to the Act, the Act of 6 November 2008 on patient rights and the Patient Ombudsman (consolidated text: Journal of Laws of 2024, item 581), and the Act of 5 December 1996 on the professions of physician and dentist (consolidated text: Journal of Laws of 2024, items 1287, 1897).

  5. The medical entity performs medical activities in the MABEL HAIR medical facility (hereinafter also referred to as: “MABEL”).

  6. Clients of MABEL HAIR CLINIC Spółka z ograniczoną odpowiedzialnością are obliged to read the applicable regulations before using the services.

§ 2

These Organizational Regulations have been created based on Article 24 in connection with Article 23 of the Act of 15 April 2011 on medical activity and define the method and conditions for providing health services by MABEL HAIR CLINIC, including:

  1. Specification of the medical entity.

  2. Goals and tasks of the medical entity.

  3. Type of medical activity and the scope of health services provided.

  4. Place of health services delivery.

  5. Organizational structure of the medical facility.

  6. The process of providing health services in the medical entity.

  7. Fees for procedures.

  8. Conditions of cooperation with other entities performing medical activities.

  9. Rules for making medical documentation available and the amount of fees for making it available.

  10. The management style, organization, and tasks of individual units and organizational cells.

  11. Rights and obligations of the Patient.

  12. Filing complaints and claims.

  13. Order regulations.

§ 3

The terms used in these Regulations shall mean:

  • Regulations: this document,

  • Medical Entity: MABEL HAIR CLINIC Spółka z ograniczoną odpowiedzialnością with its registered office in Warsaw, ul. Świętokrzyska 30/63, 00-116 Warsaw, NIP: 5253013662, REGON: 529404400, KRS 0001121972,

  • Manager: Management Board of MABEL HAIR CLINIC Spółka z ograniczoną odpowiedzialnością with its registered office in Warsaw,

  • Act: the Act of 15 April 2011 on medical activity (consolidated text: Journal of Laws of 2025, item 450),

  • Medical Documentation: medical documentation within the meaning of the provisions of the Act of 6 November 2008 on patient rights and the Patient Ombudsman (consolidated text: Journal of Laws of 2024, item 581),

  • Patient/Client: a person who requests the performance of health services or uses health services at MABEL HAIR CLINIC,

  • Staff: persons who provide work at MABEL HAIR CLINIC based on an employment contract, cooperation agreement, or civil law contract concluded with them,

  • Price List of Services: the current price list of services and goods in the offer of MABEL HAIR CLINIC expressed in Polish zlotys, available on the Clinic's website,

  • Procedure: qualification, performing necessary tests, and preparation for a hair transplant or performing the hair transplant procedure,

  • Post-procedure: care activities and possible examinations performed after a hair transplant.

Goals and Tasks

Goals

The goal of MABEL HAIR CLINIC is to organize and provide outpatient health services in the field of aesthetic medicine, as well as to promote health and advocate pro-health behaviors by qualified medical staff and trained auxiliary staff, who hold the qualifications required by law, as well as to perform other medical activities resulting from the treatment process or separate regulations governing the rules of their use for patients requiring outpatient services.

Tasks

The task of the medical entity MABEL HAIR CLINIC is in particular:

  1. compliance with professional ethics and patient rights,

  2. providing specialized outpatient health services in the field of aesthetic medicine, with due diligence and safety principles,

  3. qualification for hair transplant procedures,

  4. performing hair transplant procedures,

  5. performing injection procedures for prevention, preservation, saving, restoration, and improvement of health,

  6. providing individual advice and consultations,

  7. educational activities, health promotion, and activities aimed at health prevention,

  8. providing high-quality medical equipment with appropriate certificates in accordance with applicable law,

  9. meeting sanitary and other standards regulated by law in force in the Republic of Poland through the premises and equipment,

  10. providing high-class specialists and other medical personnel.

Type of Medical Activity and the Scope of Health Services Provided

  1. The medical entity MABEL HAIR CLINIC conducts medical activity in the scope of outpatient health services.

  2. Health services are provided within the scope of the following healthcare functions: [7110] Collection point for test materials and [9450] Diagnostic and treatment room.

Place of Health Services Delivery

The medical entity MABEL HAIR CLINIC Spółka z ograniczoną odpowiedzialnością provides health services at its registered office located at ul. Sienna 83/83, 00-815 Warsaw.

Organizational Structure of the Medical Facility

  1. MABEL HAIR CLINIC performs its activity through a set of property components in the form of a medical facility (MABEL HAIR), which constitutes an organizational unit, i.e., a separate part of the medical facility.

  2. The organizational unit of the medical facility is divided into organizational cells.

  3. The organizational cells of the organizational unit of the medical facility are: Collection point for test materials [7110], Diagnostic and treatment room [9450], Surgical clinic [1500].

Course of the Health Services Delivery Process

§ 1 General Information

  1. In the entity, health services are provided exclusively by persons practicing a medical profession or other persons holding appropriate qualifications and authorizations, and meeting the health requirements specified in separate regulations.

  2. The medical entity provides health services with due diligence, in accordance with the indications of current medical knowledge, respecting patient rights and in accordance with the principles of professional ethics.

  3. The premises and equipment of the medical entity with medical gear comply with professional, sanitary requirements, and standards specified in separate regulations.

  4. The apparatus and medical equipment of the medical entity are placed on the market and put into use in accordance with applicable regulations.

  5. Before and during the provision of health services, the patient has the right to obtain all necessary explanations regarding the procedure being performed.

§ 2 Basic Rules of Performing Procedures

  1. The catalog of currently available procedures is available at the facility and on the Clinic's website.

  2. MABEL HAIR CLINIC reserves the right to make changes to the Clinic's offer.

  3. Using the services of MABEL HAIR CLINIC is possible after the Client has been qualified for the procedure by the professional staff of the Clinic, completed the appropriate forms, including a questionnaire regarding their health condition, the Patient Card, Consent to the procedure, Consent to the processing of personal data, and made payment for the services.

  4. The course of the procedure includes: trichological consultation; initial consultation, during which the potential patient is informed about the characteristics of the hair transplant procedure, the steps of its implementation, possible complications, methods of care, and recovery period, the number of needed grafts and the valuation are also estimated; proper consultation consisting in detailed determination of the number of grafts, place of transplantation, detailed medical examination to establish any contraindications to the procedure, taking hair for analysis and possibly taking blood for laboratory tests; in case of positive qualification, referral of the patient to a hair transplant clinic in Turkey or another EU member state or performing the procedure at MABEL HAIR CLINIC in Poland; post-procedure consultation and performing post-procedure treatments consisting in washing the scalp, possible PRP injection, scalp massages, and applying conditioners.

  5. The Clinic's offer is intended for persons who are at least 18 years old and have full legal capacity. Minors and persons with limited legal capacity may use the Clinic's services only in the event of consent, in writing, by a parent or legal guardian.

  6. An adult Client may consent to the procedure, and in the case of persons under 18 years of age, their legal representative.

  7. MABEL HAIR CLINIC is entitled to verify the identity of the Patient directly before the scheduled visit or procedure by presenting an identity card, passport, or other document confirming identity.

  8. Before proceeding with the service, the Client is made aware of health contraindications and possible indications for a given procedure.

  9. Clients are obliged to inform the employee about any deterioration in their well-being during the performance of the service.

  10. In case of doubt as to the safety of performing the procedure, the person performing the procedure has the right to refuse to conduct it or to interrupt its execution.

§ 3 Scheduling Visits

  1. Procedures are performed only during the operating hours of the facility, i.e., from Monday to Friday from 09:00 to 21:00 and on Saturdays from 09:00 to 16:00, only after prior reservation made by the Client.

  2. MABEL HAIR CLINIC reserves the right to change the working hours of the Clinic, where this change may mean temporary closure of the facility, as well as shortening or extending its opening hours.

  3. MABEL HAIR CLINIC undertakes to inform Clients scheduled for a visit about the changes indicated above.

  4. The Client has the right to complete the purchased service at another nearest possible date, if the above circumstances affected the date of their visit, and in case of impossibility to complete the selected service, the Client is entitled to a refund of the service costs, if such were paid.

  5. Registration of Patients and scheduling dates for health services belong to the tasks of the Clinic's Staff.

  6. Patient registration takes place during the Clinic's opening hours: in person at the facility, by legal guardians or third parties if they are authorized to do so; by phone at 792 122 028; via email at mabelhairclinic@gmail.com.

  7. In order to schedule a visit, the Clinic's Staff is authorized to request the following information from the Patient or the person scheduling: first and last name, PESEL number, home address, telephone number, email address.

  8. The method of processing the Patient's personal data by the Medical Entity is described in detail in the document entitled “Privacy Policy”, available in the Clinic and on the website.

  9. A Client who has reserved a date is obliged to arrive at the Clinic no later than 15 minutes before the agreed hour in order to complete all formalities.

  10. A Client who has made a reservation for a subsequent procedure is obliged to arrive at the Clinic no later than 10 minutes before the agreed hour in order to properly prepare for the planned procedure.

  11. In case the Client is late for the agreed date, the lateness time may be deducted from the time provided for the procedure, unless the full execution of the service in such time would be impossible or risky. In such case, MABEL HAIR CLINIC has the right to refuse to perform the procedure in its entirety.

  12. The scheduled procedure can be canceled without any consequences, including financial ones, at the latest 24 hours before the scheduled reservation time.

  13. MABEL HAIR CLINIC reserves the right to charge the Client for the service according to the Price List in force at that moment, if the cancellation of the procedure by the Client occurs less than 24 hours before the scheduled time or if the Client does not show up for the planned visit without canceling it beforehand.

  14. MABEL HAIR CLINIC allows the possibility of changing the agreed date by the Client in particularly justified cases, with the consent of the person who was to perform the procedure.

§ 4 Refusal to Perform the Procedure

MABEL HAIR CLINIC reserves the right to refuse to perform the service in the event that:

  1. The Client refused to provide data for the procedure registration or to sign documents necessary to conduct the procedure, including consent to its execution.

  2. The Client refused to read and sign the document regarding the processing of their personal data by the Medical Entity.

  3. The Client's condition indicates consumption of alcohol or other intoxicants.

  4. There is a contraindication to the procedure.

  5. The Client behaves aggressively or contrary to the rules of social coexistence.

  6. The Client made a reservation for a procedure at the Clinic twice and failed to show up twice without canceling the reservation.

Fees for Procedures

  1. The medical entity provides health services on a paid basis, in accordance with the Price List in force on the day of the service.

  2. Health services provided in the Medical Entity are not reimbursed by the National Health Fund.

  3. The Price List for health services is available at the Clinic and on the Clinic's website.

  4. The Clinic is authorized to change the price list for health services.

  5. The final price of a specific service is determined during consultation with a specialist and may differ from that specified in the Price List, however, in each case, its individual agreement with the Client takes place before starting to provide the service.

  6. The Client pays the fee for the procedure before the procedure or, exceptionally, directly after the completed procedure.

  7. If additional services were provided during the visit, the Client is obliged to report to the reception desk to pay the balance.

  8. The Clinic accepts the following forms of payment: payment card, cash, bank transfer.

  9. In the case of payment by bank transfer, the patient should show proof of transfer.

  10. Purchased procedures or packages of procedures are non-refundable.

  11. Purchased procedures or packages of procedures must be used within a period not exceeding 3 months from the date of purchase.

  12. After the expiry of the above period, the purchased procedures or packages of procedures expire and cannot be used.

Cooperation with Other Medical Entities

  1. In order to ensure the correctness of patient treatment and continuity of the process of providing health services, MABEL HAIR CLINIC cooperates with other entities performing medical activities or providing health services for these patients, including entities performing medical activities outside the territory of the Republic of Poland.

  2. Cooperation with other entities performing medical activities takes place with respect for applicable laws and with respect for patient rights.

  3. Supervision over the correctness of the process of health services provided by other entities is exercised by the Manager of the Medical Entity.

Medical Documentation

§ 1 Basic Rules

  1. MABEL HAIR CLINIC maintains medical documentation of persons using health services and ensures the protection and confidentiality of data contained in this documentation in accordance with commonly applicable laws.

  2. For health reasons, a Client scheduled for a procedure during their first visit is asked to fill out: a questionnaire regarding their health condition and the Patient Card; in the case of an aesthetic medicine procedure, consent to the procedure (in the case of Patients under 18 years of age, the consent is completed by a legal guardian); consent to the processing of personal data; other documents adapted to individual procedures.

  3. MABEL HAIR CLINIC reserves the right to refuse to perform the procedure without prior completion and signature of the aforementioned documents.

  4. Responsibility for the correctness and truthfulness of information entered in all of the above documents lies solely with the Client.

  5. MABEL HAIR CLINIC is not liable for damages and post-procedure complications resulting from the Client concealing contraindications to the procedure.

§ 2 Making Medical Documentation Available and Storing It

  1. The Patient's medical documentation is the property of MABEL HAIR CLINIC.

  2. The Patient has the right of access to the documentation concerning their health condition and the services provided to them.

  3. Medical documentation may be made available upon written request to: the Patient whom the documentation concerns; the Patient's legal representative; a person authorized by the Patient to obtain medical documentation, upon presentation of identity document; entities authorized to obtain medical documentation based on applicable laws.

  4. After the Patient's death, medical documentation is made available to a person authorized by the Patient during their lifetime or to a person who, at the time of the Patient's death, was their legal representative.

  5. Medical documentation may be made available: for inspection at the Clinic; by preparing extracts, transcripts, copies, or printouts; by issuing the original against confirmation of receipt and subject to return after use, at the request of public authorities or common courts; via electronic communication means; on an electronic data carrier.

  6. Access to medical documentation or issuing its extracts, transcripts, copies, or printouts takes place on the basis of an application submitted by the Patient, their legal representative, or a person authorized by them.

  7. The application will be considered each time by the Manager of the Medical Entity immediately, no later than within 7 days from the date of submission of the application.

  8. Inspection of the documentation or its collection will be possible after prior determination of the date and hour of inspection or collection with an employee of the Medical Entity.

  9. Making documentation kept in electronic form available may occur by transferring an electronic data carrier with the recorded documentation or in the form of a paper printout certified for compliance with the electronic documentation.

  10. Medical documentation may be sent by email to the specified email address.

  11. Making medical documentation available occurs in a manner ensuring confidentiality and protection of personal data.

  12. The decision to make medical documentation available is made by the Manager of the Medical Entity.

  13. In the event that making the documentation available is not possible, the refusal along with the reason must be in writing.

  14. The Clinic and persons providing health services on its behalf may provide other entities performing medical activity with information related to the Patient when: the Patient or their legal representative consents to the disclosure of secret; keeping the secret may pose a danger to the life or health of the Patient or other persons; there is a need to transfer necessary information about the Patient to other persons practicing a medical profession, participating in the provision of services; applicable laws so provide.

  15. The Clinic does not charge any fees for preparing an extract, transcript, or copy of medical documentation, both in paper version and on an electronic data carrier.

  16. Medical documentation is stored in the premises of the Clinic's registered office. Administrative and employee documentation is stored in the premises of the registered office of MABEL HAIR CLINIC, and archive, accounting documents, and HR documentation are stored by an external accounting office.

Entity Management and Organizational Cells

  1. The activity of MABEL HAIR CLINIC Sp. z o.o. is managed by the Management Board of the Medical Entity, in accordance with the representation rules resulting from the articles of association and registration in the KRS, being the Manager of the entity within the meaning of the provisions of the Act.

  2. The Manager manages the medical entity and represents it outside, makes decisions regarding the functioning of the Entity independently, and bears responsibility for it.

  3. The Manager is entitled to all obligations and rights that commonly applicable laws impose on a medical entity, including the obligations and rights of an employer towards the employed Staff.

  4. The competencies of persons managing the Medical Entity include, among others: representing the Entity outside, running current affairs, ensuring proper functioning of the Entity, coordinating and supervising the work of cells and employees, shaping a positive image of the company, making decisions on economic cooperation and personnel matters, and issuing internal regulations.

  5. In order to support management activities, the Manager may employ or conclude civil law contracts with administrative and office staff, nurses, cosmetologists, and other staff, as well as conclude contracts with doctors. The Staff is subject to the supervision of the Manager of the Medical Entity.

  6. The tasks of the Reception employees include: organizing the current work of the reception, patient service, correct and timely settlement of services and collection of fees, answering phones, booking patients for visits and informing about the scope of services, and other tasks commissioned by the Manager.

  7. The basic tasks of the Collection Point include, in particular, taking blood for laboratory tests.

  8. The basic tasks of the diagnostic and treatment room include, in particular: providing comprehensive medical care within specialized outpatient treatment, qualification for hair transplantation, examination, diagnosis, medical advice, treatment and prevention of diseases constituting a potential contraindication to the procedure, ruling on the state of health, referring for additional tests, taking hair samples, PRP injections, performing the hair transplant procedure, referring patients to treatments in another EU country or Turkey, and keeping documentation.

  9. The basic tasks of the surgical clinic include, in particular: performing minor surgical procedures, including some stages of the hair transplant procedure or corrections after transplantation, post-procedure care (monitoring healing, removing dressings and sutures, care and pharmacological recommendations), monitoring treatment effects, and educating patients in the scope of recovery and hair care.

  10. Individual units and organizational cells as well as persons working in them are obliged to cooperate to ensure smooth, effective, and lawful functioning of the medical entity.

Rights and Obligations of the Patient

While using the services of the Clinic, the Patient has the right to:

  • obtain accessible information about their health condition, diagnosis, proposed and possible diagnostic and treatment methods, foreseeable consequences of their use or omission, treatment results, and prognosis,

  • express consent or refuse to express consent to the examination or provision of health services, where in the case of surgical procedure or method involving increased risk, consent is expressed in writing,

  • respect for dignity and polite treatment by the Clinic's Staff,

  • keeping secret information related to them, obtained in connection with the practice of a medical profession,

  • information about patient rights specified in the Act on patient rights and the Patient Ombudsman,

  • information about the type and scope of health services provided by the entity,

  • access to medical documentation regarding their health condition and health services provided to them.

The Client's duties include:

  • compliance with these Regulations of the Clinic,

  • having a valid identity document,

  • compliance with personal hygiene rules,

  • following recommendations given by the Clinic's Staff,

  • compliance with order regulations described in these Regulations.

Filing Complaints, Claims, and Requests

  1. Patients may submit claims, complaints, and requests regarding the Clinic's activities to the Management Board of MABEL HAIR CLINIC.

  2. Claims, complaints, and requests can be submitted only in writing: directly at the facility, by registered mail to the address of the registered office, or by email to mabelhairclinic@gmail.com.

  3. Complaints, claims, and requests will be considered immediately, at the latest within 30 days.

  4. Clients submitting a claim will be informed about the decision in writing or electronically, depending on the form in which the request was received.

  5. In the event of acceptance of the claim and the need to refund funds, the medical entity refunds the payment using the same payment method that the patient used, unless the patient has explicitly agreed to another refund method that does not involve any costs for them.

Order Regulations

Patients are obliged to:

  • comply with the ban on bringing and consuming alcoholic beverages, intoxicants, smoking tobacco products and electronic cigarettes on the premises of the Clinic,

  • leave their clothes in the designated place for the duration of the health services,

  • turn off electronic devices, in particular mobile phones, for the duration of the health service,

  • respect the property of the Clinic (damages caused by the fault of the Patient will be removed at their expense) and the property of other Patients,

  • keep clean in the places they use,

  • comply with social behavior standards and respect the dignity and privacy of other Patients and Staff,

  • comply with the recommendations of the Staff as well as safety rules and fire regulations,

  • not start or use medical equipment and apparatus without authorization,

  • not disturb the provision of healthcare services.

The Clinic is not responsible for valuable items belonging to Patients left on hangers, in lockers, or premises of the Clinic. It is recommended to leave valuable items at home.

Final Provisions

  1. Each Patient using the services of MABEL HAIR CLINIC is obliged to comply with the provisions resulting from these Regulations.

  2. In matters not regulated by these Regulations, relevant provisions of the Civil Code and other commonly applicable regulations shall apply. Any disputes arising from the performance of obligations related to a given service will be resolved by the court competent for the registered office of MABEL HAIR CLINIC.

  3. MABEL HAIR CLINIC reserves the right to modify the provisions of these Regulations, provided that the changes introduced will not affect the rights acquired by patients before the entry into force of the changes.

  4. The currently applicable and unified text of the Regulations is available on the Clinic's website and at the Reception Desk.
