2. Scope and purposes of data processing
We process the data provided in the contact form (first and last name, email address, phone number, message content) in order to respond to your inquiry and schedule a consultation. The data may also be processed for the purpose of providing medical services, in accordance with separate regulations regarding medical documentation.
3. Legal basis
We process data pursuant to Article 6(1)(b) of the GDPR (actions aiming at the conclusion of a contract), Article 6(1)(f) of the GDPR (legitimate interest of the controller, which is handling correspondence), and, with regard to health data, Article 9(2)(h) of the GDPR.
4. Retention period
We store data from the contact form for the period necessary to handle the inquiry, for no longer than 12 months after the end of the correspondence. Medical records are stored for the periods required by law.
5. Data recipients
Data may be entrusted to entities that support us technically (website hosting, e-mail) on the basis of entrustment agreements. We do not sell personal data and we do not transfer it to third parties for marketing purposes.
6. User rights
You have the right to access, rectify, erase, or restrict the processing of your data, the right to object, and the right to lodge a complaint with the President of the Personal Data Protection Office. To exercise your rights, please contact us by email.
